We have written these terms in plain language on purpose. They are here to be clear about what this website is, what it is not, and what you can expect from us.
Everything on this site, including service pages, blog articles, videos, downloadable guides and answers to common questions, is general information about our clinic and about movement, injury and recovery. It is not medical advice, it is not a diagnosis, and it is not a substitute for an assessment by a regulated health professional who has examined you.
Reading this site does not create a clinician-patient relationship with Achieve Movement or any of our clinicians. That relationship begins when you attend an assessment and consent to care.
If you think you have a medical emergency, call 911 or go to your nearest emergency department. Symptoms after a head injury such as worsening headache, repeated vomiting, confusion, seizures or loss of consciousness need urgent medical attention.
Do not delay seeking care or ignore professional advice because of something you read here. Every person and every injury is different. Nothing on this site promises a particular result. Our clinicians will discuss what is realistic for you at your assessment.
The booking calendar on this site is for a discovery call: a short, no-charge conversation with our team about what you are dealing with and whether we are the right fit. It is not a treatment appointment, and no assessment, diagnosis or treatment is provided on a discovery call.
When you book, you agree to provide accurate contact information and to be contacted by phone, email or text about your request. Please keep detailed health information for your assessment rather than the booking form.
Treatment appointments are scheduled, changed and cancelled directly with the clinic by phone at (519) 909-3475 or by email at [email protected]. This lets our team match you with the right clinician and manage the schedule fairly for every patient.
Our appointment, cancellation and missed-appointment policies, including any fees for late cancellations or missed visits, are provided to you when you book and at your first visit. By booking an appointment you agree to those policies.
Fees are payable at the time of service unless other arrangements have been made. We direct bill most extended health insurance plans; whether direct billing is available depends on your plan and policy, and you remain responsible for any amount your insurer does not cover. We also support Workplace Safety and Insurance Board (WSIB) and motor vehicle accident claims; coverage decisions for those claims are made by the insurer or the WSIB, not by our clinic.
We offer video appointments for people who cannot attend in person. Virtual care is available to Ontario residents and is appropriate only for the kinds of assessment and follow-up that can be done safely by video. Our clinician will tell you if an in-person visit is needed instead.
For video appointments you are responsible for having a private space, a stable internet connection and a working camera and microphone. Virtual care is not for emergencies.
By contacting us or booking through this site, you consent to receive communications about your request, your appointments and your care. We send marketing messages only with your consent, and every marketing email or text includes a way to unsubscribe. The content on this site, including text, photographs, video, graphics, the Achieve Movement name and logo, and the layout of the site, belongs to Achieve Movement Health Clinic or is used under licence. You may view, download and print pages for your own personal, non-commercial use. You may not copy, republish, sell or redistribute our content, or use our name or logo, without our written permission. Linking to our pages is welcome, as long as the link is accurate and does not suggest an endorsement we have not given.
You agree to use this site lawfully and respectfully. You may not attempt to gain unauthorized access to the site, its booking system or its underlying platform; interfere with its operation or security; submit false booking information; scrape or harvest content or data; or use the site to send spam or unlawful material.
This site uses services provided by others, including our website and booking platform (HighLevel), an embedded Google Map, a display of public Google reviews, and links to our social media profiles. Those services are governed by their own terms and privacy policies. We do not control them and are not responsible for their content, availability or practices. A link from our site is not an endorsement.
Reviews shown on this site are public Google reviews written by patients and published by Google under Google's terms. If you choose to leave a review, you are responsible for its content. We use patient names, stories, photos or video on this site only with that person's documented consent.
If you send us feedback, ideas or suggestions about our services or this site, you agree we may use them without obligation to you.
We work to keep the information on this site accurate and current, including our services, team and hours, but details can change. Please confirm anything important with the clinic directly. These terms are governed by the laws of the Province of Ontario and the federal laws of Canada that apply in Ontario. Any dispute about this website will be dealt with in the courts of Ontario.
