Achieve Movement Health Clinic ("Achieve Movement," "we," "us," "our") operates the website at achievemovement.ca and provides chiropractic, physiotherapy and movement-based care from our clinic at 63 Queensway W, Simcoe, Ontario.

This policy explains what information we collect through this website, how we use it, who we share it with, and the choices you have. It also explains, in plain terms, how your health information is handled once you become a patient, because we know that is the question people actually have.

The short version

The website collects only what you give us (for example, your name, email and phone number when you book a discovery call) and standard usage data through cookies.
Your health records are not collected through this website. They are created at the clinic, kept in our secure practice-management system, and protected under Ontario's health privacy law.
We never sell your personal information.
We use Google Analytics, Meta Pixel and our website platform (HighLevel) to run and measure this site. A cookie banner lets you accept or decline non-essential tracking.
You can ask us what we hold about you, ask us to correct it, or withdraw your consent to marketing at any time.

Who is responsible for your information

Achieve Movement Health Clinic is a health information custodian under Ontario's Personal Health Information Protection Act (PHIPA). Our clinicians are regulated by the College of Chiropractors of Ontario and the College of Physiotherapists of Ontario. For information collected through this website and our marketing, Canada's Personal Information Protection and Electronic Documents Act (PIPEDA) and Canada's Anti-Spam Legislation (CASL) also apply.

Information we collect

1. Information you give us through the website

When you book a discovery call, request a callback, or contact us, we collect the details you enter: typically your name, email address, phone number, and a short note about what you are dealing with. When you call, email or text the clinic, we keep the information needed to respond to you and to book or change appointments.

Please do not include detailed personal health information in website forms, chat or email. A brief description of your goal is enough for us to respond. Health details belong in your assessment, where they are collected and protected as part of your clinical record.

2. Information collected automatically

Like most websites, we use cookies and similar technologies to understand how the site is used. This can include your IP address, device and browser type, the pages you visit, how you arrived at the site, and the approximate region you are visiting from. Details are in the cookies section below.

3. Your health information

Once you become a patient, your clinical record is created at the clinic. It includes your health history, assessment findings, treatment plan, progress notes, and billing and insurance details. This information is collected directly from you, with your consent, and is stored in our practice-management and health-records system (Jane), which is separate from this website.

We collect, use and share health information only for the purposes of providing you with care, for billing, and as otherwise permitted or required by law.

How we use your information

To respond to you, book your discovery call, and schedule or change appointments.
To provide, plan and document your care.
To bill for services, including direct billing to your extended health insurer, and to support Workplace Safety and Insurance Board (WSIB) and motor vehicle accident claims when you ask us to.
To send appointment confirmations, reminders and practical information about your care.
To send helpful movement and injury-prevention content, clinic news or offers, only if you have agreed to receive them. You can unsubscribe at any time.
To understand how this website is used and to improve it.
To measure the results of our online advertising.
To meet our legal, regulatory and professional obligations.

Email and text messages

If you give us your email address or phone number, we may use it to communicate with you about your request, your appointments and your care.

We only send marketing emails or texts with your express consent, as required by Canada's Anti-Spam Legislation. Every marketing message includes an unsubscribe option, and you can also opt out by contacting the clinic.

If you agree to receive text messages, message frequency varies and standard message and data rates from your carrier may apply. Reply STOP to any text to stop receiving messages, or HELP for assistance. We do not share your mobile number with third parties for their own marketing.

Cookies and tracking technologies

A cookie is a small file stored on your device by your browser. We use cookies and similar tools for three purposes: to make the site work, to understand how it is used, and to measure our advertising.

When you first visit, a cookie banner asks for your choice about non-essential cookies. Essential cookies that keep the site and its booking calendar working are always on. You can change your choice at any time through the banner or your browser settings.

Meta Pixel (Meta Platforms, Inc.), which helps us measure whether our Facebook and Instagram ads bring people to this site and lets us show relevant ads to people who have visited. You can adjust ad preferences in your Facebook and Instagram settings.
HighLevel (also known as LeadConnector), our website and customer-relationship platform, which hosts this site, the discovery-call calendar and our email and text messaging. It sets cookies needed for the site and calendar to work and to connect your booking to your contact record.
Google Maps, which is embedded on our contact page and may set its own cookies when the map loads.
Google Reviews, which we display on this site through our platform. The reviews themselves are collected and published by Google under Google's terms.

Each of these providers has its own privacy policy that governs how they handle information they collect. Blocking cookies in your browser may affect how parts of the site work, including the booking calendar.

We do not sell, rent or trade your personal information. We share it only in these situations:

Service providers who help us operate the clinic and this website, including our website and communications platform (HighLevel), our practice-management system (Jane), our analytics and advertising tools (Google, Meta), and our payment processors. These providers may only use your information to provide their services to us.
Insurers and claims organizations, when you ask us to direct bill your extended health plan or to support a WSIB or motor vehicle accident claim. We share only what the claim requires.
Other health-care providers involved in your care, such as your family doctor or a specialist, with your consent or as permitted by PHIPA.
When required or permitted by law, for example to respond to a court order, a regulatory college, or a public-health requirement.

Some of our service providers store information on servers outside Canada, including in the United States. When that happens, the information may be subject to the laws of that country. We choose providers with appropriate safeguards and share only what is needed.

Reviews, testimonials and photos

Reviews shown on this site are public Google reviews. If you leave a review on Google, it is governed by Google's terms and is visible to the public.

We only use a patient's name, story, photo or video on this website or in our marketing with that person's documented consent, which can be withdrawn at any time by contacting the clinic.

How we protect your information

We use reasonable physical, technical and administrative safeguards to protect the information we hold, including secure practice-management software, access limited to staff who need it, and staff training on privacy. No method of transmission or storage over the internet is completely secure, so we encourage you to share health details in person or by phone rather than through website forms.

How long we keep it

Contact information from website enquiries is kept for as long as needed to respond to you and to meet our record-keeping obligations. Clinical records are kept for the minimum periods required by our regulatory colleges and Ontario law, after which they are securely destroyed.

Your rights and choices

You have the right to:

Access the personal and health information we hold about you, and receive a copy.
Correct information that is inaccurate or incomplete.
Withdraw consent to marketing communications at any time.
Limit how your health information is shared with others involved in your care, subject to the limits set by law.
Ask questions or raise a concern about our privacy practices.

Children and youth

We treat young athletes and children as part of our concussion and sports-injury care. Bookings and consent for patients under 16 are handled by a parent or guardian. We do not knowingly collect personal information from children through this website.

Virtual care

If you use our video appointments, the session runs through secure software chosen for clinical use. You are responsible for the privacy of the location and device you use on your end. Virtual care is available to Ontario residents.

Links to other websites

This site links to other websites and services, such as Google Maps and our social media profiles. We are not responsible for their content or privacy practices. Please review their policies before providing any information.

Changes to this policy

We may update this policy as our practices or the law change. The date at the top shows the latest revision. Material changes will be posted on this page.
